"Family Information Form"

Email is a vital component of the communication between the RSMS Band and parents.  Notes home from teachers often end up on the hallway floors or crumpled in the backpacks, rather than actually getting home to Mom and Dad!  So we are asking every family to complete the following form which will be compiled into a band database for band use only.

If you have more than one student in band, please go ahead and fill out the TOP portion of the form for EACH student (you will need to re-start the form for each student), but you only need to complete the bottom section once. 

Please PROOFREAD your information (especially the email addresses) to be sure there are no typos.  THANK YOU!

ADULT VOLUNTEER NEEDS:  Although Rising Starr Band does not have an officially organized Band Booster organization, parent help is needed throughout the year.  

For this section, please help us by placing the following in the box, rather than just an "X":

    M = MOM (or female guardian) is interested in helping
    D = DAD (or male guardian) is interested in helping
    B = BOTH are interested in helping

     Help with band picture day

     Help with monthly "Moe's Night" ...
(publicity flyers posted at school, announcement
                        submitted to office, signs outside school, take a shift at the restaurant to remind families to make
                        their affiliation with RSMS known to Moe's staff, etc.... Hopefully enough volunteers will sign up
